Care crafted for seniors.
As we age, our health care needs become more complex. We can have more health problems. We might be less mobile. And it’s easier to become isolated. Our OASIS program was developed to address the challenges of aging and help older people with mental illness live full, safe, independent lives. We partner with our clients, their families and their support providers to help them create a loving safety net of care and community.
-
We’ll come to you.
We make it easy to get care by coming to wherever our clients live: in their homes, in assisted living facilities, or in nursing homes.
-
A caring team of experts.
We know something about caring for older adults with mental illness because we’ve done it for more than four decades. Our OASIS team includes doctors, therapists, and case managers, who work together to support all facets of our clients’ lives.
-
Connections created.
Sometimes the best medicine is having someone your own age to talk to. Our peer support program pairs older clients with someone near their age, who has achieved their own recovery, for conversation and outings.
-
A supportive home base.
From our assisted living facility with 24-hour support to a housing program that provides onsite support seven days a week, we help seniors with mental illness stay in the community in the least restrictive setting and maintain their quality of life.
